Description:
Hospice Care Coordinator will be responsible for coordinating hospice care by scheduling patient visits, managing medication orders and refills, and communicating with the interdisciplinary team to ensure smooth care transitions. Assists with patient and family inquiries, tracks hospice supplies, and supports compliance with hospice regulations.
Duties and Responsibilities:
Ensure quality service and participate in care coordination to facilitate proper communication with team members, patients, physicians, referral sources, and payer sources. Exemplify leadership in promoting and maintaining standards for giving high quality care by all members of the patient team Keep staff informed of condition updates/changes via appropriate means of communication Review clinical records and documentation to ensure regulatory and agency compliance Provide back up support to staff; supervise and teach in order to improve practice with the agency Coordinate the quarterly clinical record review program and participate in Performance Improvement initiatives Maintain medication and DME updates/orders as necessary to meet individual needs of patients Document and maintain current data in Electronic Medical Record (EMR) Maintain current practice knowledge through internal and external training programs/seminars and act as a resource for the interpretation of clinical practice/standards Perform other duties as assigned
Requirements:
Current Pennsylvania LPN license Hospice background preferred Computer / EMR savvy Ability to work independently as well as collaboratively with team members and manage time effectively and efficiently Strong interpersonal and exceptional customer service skills Positive, proactive attitude Why Choose Traditional Home Health Care?
