Provides skilled nursing care in accordance with physician’s orders, the patient’s care plan, and agency policies. Administers medications and treatments as prescribed. Performs patient assessments and monitors changes in patient condition. Communicates effectively with patients, families, and other healthcare team members to ensure quality care. Educates patients and families on care plans and safety measures. Documents all patient care activities in compliance with agency policies. Familiarity with Feeding Tubes, Assisted breathing Devices (BiPAP) and Hoyer Lift.

Requirements Job Requirements:

Graduate of an accredited LPN program Current LPN license in Pennsylvania Minimum of one year experience in home healthcare or a similar clinical setting CPR certification Strong communication and interpersonal skills Self-motivated with the ability to work independently Reliable transportation and valid Driver’s License Proof of
