Videos Duties The Clinical Associe LPN works in the primary care clinic. The LPN/LVN serves as a one of the core members of the Pient Aligned Care Team (PACT). The LPN/LVN works in collaborion with the Primary Care Provider (PCP), Care Manager Registered Nurse (RN) and the Medical Support Assistant (MSA). Together, with the pient and their caregiver support system, they work to manage the care of a panel of adult pients across the continuum. LPN/LVN responsibilities include, not limited to: Technically proficient in initiing, performing, and completing assigned duties in providing care to variable pient populions. Knowledge and ability to appropriely carry out assigned pient care based on the pents’ conditions; to use judgment in selecting the approprie order and sequence of procedures and trements; and to accurely recognize, report and record relevant pient informion. Completed work should need only a general review by a registered nurse or physician for approprieness and conformity with established policies/procedures. Ability to observe, identify and respond to the pient’s needs for care including medicion equipment-assisted care and (pient/family educion). In organizing and delivering care, the LPN/LVN recognized and considers emotional, cultural, spiritual, socio-economic, and age-reled factors. Prepares and administers prescribed medicions (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs trements according to established policies/procedures. Observes for physical and/or emotional changes in pient’s condition from prescribed medicion/trements, promptly and accurely documenting noted changes, and reporting any deviions from normal to RN or MD/DO. Knowledge and ability to recognize urgent and emergent pient care situion, seek assistance of the RN and/or MD/DO, and initie approprie emergency interventions as directed. Knowledge and understanding of human behavior, pient motivions and reactions to situions, and ability to appropriely utilize this knowledge in working effectively with pients, family members, and other staff. Preferred Experience:
2+ years of LPN experience in an outpient or ambulory care setting Experience using Electronic Health Records (EHR) Proficiency with common outpient procedures including: Taking and documenting vital signs Specimen collection and labeling EKGs and point-of-care testing Familiarity with fast-paced, high-volume clinic environments, managing multiple pient appointments and tasks per shift. Grade Determinions :
In addition to the basic requirements sted above, the following qualificion criteria must be met for each grade. The candide’s qualificions must clearly demonstre the level of competence required for the grade. GS-03 None beyond the basic requirements. LPNs this grade level are expected to perform all duties within the scope of their license (e.g., medicion administrion); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved. GS-04 (a) Six months of qualifying experience as an LPN or LVN; or(b) Graduion from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience th involved nursing care work in a hospital, outpient clinic, nursing home, or other supervised medical, nursing, or pient care facility th provided a practical knowledge of human body structure and sterile techniques and procedures; or (c) Graduion from an approved school of least 24 months durion. Demonstred ability to serve as a responsible member of the nursing team and interact in an approprie manner with pients, family members, professional and other supportive personnel involved in the delivery of pient care, incorporing acceptable, established customer service standards into practice. Knowledge and skill sufficient to prepare, administer, and appropriely document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medicions as permitted by approved local facility policies and procedures.
