Licensed Practical Nurse (LPN) –

Homecare Job Summary:

We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide high-quality nursing care to patients in a homecare environment. The LPN will work under the supervision of a Registered Nurse (RN) or physician, assisting with patient care plans, medication administration, and health monitoring to support patients in maintaining their well-being at home.

Key Responsibilities:

Provide direct nursing care to patients in their homes, including wound care, catheter care, injections, and medication administration. Monitor patients’ health status and report changes to the RN or physician. Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support. Educate patients and families on health conditions, medications, and proper care techniques. Document patient care and observations accurately and in a timely manner. Collaborate with other healthcare professionals to ensure coordinated care. Maintain infection control and safety standards in the home environment. Follow established care plans and agency protocols.

Qualifications:

Current and valid LPN license in the state of practice. CPR certification (Basic Life Support). Minimum of 1 year of nursing experience; home health experience preferred. Strong clinical, communication, and organizational skills. Ability to work independently and adapt to a home setting. Reliable transportation and a valid driver’s license.

Working Conditions:

Travel required to patients’ homes. Flexible hours, including evenings or weekends, depending on patient needs.
