RN/LPN Oak Hollow of Georgetown 2715 South Island Road, Georgetown, SC 29440 Job Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will be responsible for providing high-quality patient care in various settings, including hospitals, clinics, and long-term care facilities. The LPN will work under the supervision of registered nurses and physicians, ensuring that patients receive the best possible care. Duties Deliver patient care with a focus on comfort and well-being. Conduct physical examinations and monitor vital signs. Practice infection control measures to maintain a safe environment for patients. Document patient information accurately in electronic health records, PCC Assesses patients by physical examination, including pertinent diagnostic testing to determine health status. Administers medications and treatments Participates in the care planning process Supervises Nursing Assistants, oversees ADLs and documentation Communicates with physicians regarding changes in resident’s conditions, diagnostic test results, etc. Documents assessments and care in compliance with standards of care and company policy. Educates patients and their families on health-related issues. Completes required forms and documents in accordance with company policy and state and/or federal regulations. Other duties as assigned. Qualifications Valid Licensed Practical Nurse (LPN) license in South Carolina. Strong skills in patient care, including catheterization and medication administration. Knowledge of acute pain management techniques and infection control protocols. Ability to perform physical examinations and manage airway issues effectively. Excellent communication skills for effective health coaching and patient interaction. Compassionate demeanor with a commitment to providing high-quality nursing care.
