The LPN (Nurse MedTech) reports directly to the WellnessDirector.
Shift DetailsLPN•Licensed Practical Nurse 12 hourshifts, Every other weekend requiredPURPOSEThe Wellness Nurse/NurseMed Tech is responsible for monitoring the health and wellbeing ofour residents and provides leadership support and assistance withresident care and services. Responsibilities include but are notlimited to medication care administration, communication withresident support services and families and maintaining clinicalquality assurance while leading, demonstrating and supporting allthe elements of wellness which are core components of Phoenixprograms and services.
PRINCIPLE DUTIES AND RESPONSIBILITIESLPN
•Licensed Practical NurseResident Care Identifies on-going needs andservices of residents through the assessment / Personalized ServicePlan (PSP) process Ensures proper follow through and documentationfor residents with a change in clinical care needs Assist theWellness Director with Completing the clinical sections of monthlywellness visits and PSP to accurately reflect the resident monthlyInforms the Wellness Director and/or Executive Director of anychanges in medication or service level that may result in a highertier level or potential move-out Assist with obtaining weights andvital signs monthly for each resident prior to completing monthlywellness health updates Assist with direct resident care asneeded
EDUCATION/EXPERIENCE/LICENSURE
•CERTIFICATIONLPN•LicensedPractical Nurse Must be a Licensed Practical Nurse or RegisteredNurse A minimum of one year of experience working with elders ordisabled individuals, preferred Must have demonstrated Leadershipcapabilities Knowledge and experience in Assisted Living industryand Dementia care or Skilled Nursing, preferred Ability to workweekends, evenings and flexible hours, available to our customersat peak service delivery days and times
