HEALTH NURSE – LPN JOB DESCRIPTION
Under the direct supervision of the Reproductive Health Care Coordinator, the Family Planning Reproductive Health Nurse will participate and will carry out responsibilities which ensure high quality, well-coordinated patient care for the Little River Medical Center family planning program as part of the South Carolina Family Planning Initiative. The objectives of the family planning program are to reduce unintended pregnancy among women, men, and teens by developing and disseminating culturally appropriate information about highly effective contraceptive methods and by increasing women’s, men’s and teens’ access to contraceptive counseling and clinical services.

Essential Duties and Responsibilities, include the following, other duties may be assigned: Daily appointment scheduling, answers incoming calls, makes patient appointment reminder calls, retrieval of medical records. Proficient in use of computers, typing, and ability to learn EMR systems and updates. Ability to establish and maintain the confidence and cooperation of patients. A commitment to continually improve performance, and to ability to sustain effort over time and in the face of obstacles. Ability to work with people from a range of cultures. Willingness to work in multiple locations at LRMC and within community. Assist LRMC patients seen by RHC team with completing Family Planning applications daily as needed. Assist primary nurse in preparing patients for provider, including greeting patients, patient check-in and check-out. Assists in conducting of health education activities as needed. Records results of all blood samples, cultures, EKG’s, etc in patient charts. Performs routine laboratory procedures, i.e., urinalysis, cultures, completing appropriate forms and record lab results. Administers medication, treatment/procedures, and immunizations as prescribed by the provider and follows Centers policies and procedures. Collect all patient documentation and confirm all essential information and back-up documentation is accurate and complete for various programs including, but not limited to: Medicare, Medicaid, health exchange, CHIP, Benefit Bank, sliding fee applications, etc.
QUALIFICATIONS

Bilingual:
English/Spanish, preferred. EDUCATION and/or
EXPERIENCE
Must be licensed as a Practical Nurse by the South Carolina Board of Nursing or North Carolina Board of Nursing and be in good standing; and one (1) year of practical nursing experience. Familiar with general office procedures; possess considerable knowledge of practical nursing practice and procedures. General knowledge of the Human Anatomy and Reproductive Health Care.
