RN or LPN• Full-Time Active Home Care’s Office Nurse assists the Home Care team with the clinical components of the Home Care program as it relates to the onboarding of new Personal Care Aides and supervision of patient-centered care in the home. The Office Nurse provides the clinical element of in-office orientation and the evaluation of skills in the home for staff providing direct hands-on client care. The Office Nurse ensures the field staff’s skills match the needs of our clients to ensure and promote safe patient care and ongoing support in the home. Do:
Besides working with an AMAZING Team, here are some of the things you will do: Visit new referrals for Home Care to ensure Active Home Care’s ability to safely provide in home care Review clinical components and post-test results with new hires at orientation Ensure clinical material provided to Personal Care Aides is clear and provide remediation and additional training as needed. Ensure new Personal Care Aides are supervised post orientation in the home and skills of the Aide match the care needs of the client Immediately report any potential concerns in the home of a client Schedule ongoing quarterly visits and communicate any changes to Care Team All clinical documentation must be submitted in a timely manner and must be accurate. Create and sign off on client Care Plans (yearly and quarterly visits) Qualifications Qualifications include: Active RN License OR Active LPN license Registered with the applicable state examining board. At least 21 years of age Solid understanding of home care and field staff disciplines Staffing Flexibility Demonstrated ability to build strong relationships High impact communication skills; both written and verbal Ability to cover an assigned area of responsibility.
