Monday•Friday 8am to 5pm.

Travelthrough service areas of Buncombe County, Henderson County,Transylvania County, and Polk County The role you’ll contribute:

Performs direct patient care of the home health patient in assignedhome settings. Meets psychosocial and physical needs, as well ascommunication and educational needs of patient and family asoutline in Plan of Care, under the supervision of the RN casemanager and/or RN Patient Care Supervisor. Adheres to regulatoryrequirements, home health and applicable agency policies,procedures, and standards. Communicates appropriately and timelywith team members about patient’s clinical condition, includingresults of diagnostic studies and symptomatology. Provides direct patient care during routine visits, under definedsupervision of RN Case Manager or RN Patient Care Supervisor, basedon the plan of care. Observation, recording, and reporting to theCase Manager and/or Supervisor and physician any reaction totreatment or changes in the client’s condition. Record legible,accurate, detailed service documentation of the care provided andcare coordination with supervising RN. Maintain competence indefined/key nursing skills, i.e., wound care, infusion therapy,disease management, adult education, medication oversight. Theexpertise and experiences you’ll need to succeed:

•DriverLicense, Valid and in

State Upon Hire IV Certified Preferredqualifications:
