Part-Time Nurse (RN/LPN) for Home Care Assessments Total Companion LLC 9 Willow Drive, Sumter, SC 29150 Total Companion Home Care is a compassionate and client-focused home care agency dedicated to providing high-quality care to individuals in need. We are seeking a dedicated Registered Nurse (RN) or LPN to perform assessments for our home care clients on a part-time basis.

Job Responsibilities:

• Conduct initial and ongoing home care assessments for new and existing clients.
• Develop and update individualized care plans based on client needs.
• Ensure compliance with state regulations and company policies.
• Communicate with clients, families, and caregivers to provide education and guidance.
• Document assessments and maintain accurate client records.
• Collaborate with the care team to ensure the highest level of service.

Qualifications:

• Active RN license or Active LPN license
• Experience in home care, geriatrics, or community health preferred.
• Strong assessment and documentation skills.
• Ability to work independently and travel to client homes as needed.
• Compassionate, patient-focused, and excellent communication skills.
