Monitor patients progress and work with the physicians to ensure quality patient care. Qualifications Active LPN license ( Connecticut LPN license•or willing to apply for one)

YOU MUST HAVE SOME OFFICE EXPERIENCE

•geriatrics or primary care/internal med preferred Bilingual in English and Spanish Ability to sit and talk on the phone for long periods of time

System:

We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop. Must live and work full time in the US•you can work from anywhere within the US but not outside. The Pay The LPN pay starts at $25.85 per hour (40hrs/week) The pay structure is a base pay plus a bonus per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter. and start making a meaningful impact with us!
