Phone Nurse Job Description The LPN will support a primary care clinic primarily in the phone room. The primary responsibility will be to receive patient phone calls. Additional duties include taking vital signs, performing phlebotomy, and assisting with patient follow-up for procedure/test scheduling or referrals if needed. Responsibilities Take calls from hospitals/nursing homes and relay appropriate information to practitioners in a timely manner. Receive patient phone calls. Utilize a variety of office and medical equipment, including but not limited to telephone, computer, fax, stethoscope, scales, thermometers, blood pressure cuff, and basic lab equipment. Assess patients’ general condition and take vital signs and weight. Ensure test results are received and relayed to patients in a timely manner. Document all interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record. Ensure an adequate stock of supplies and well-maintained equipment. Monitor and ensure accuracy of recording on patient medical records. Maintain all required reports, records, statistics, etc. Maintain confidentiality in matters regarding the practice, its patients, and employees.
