Job Description:
Provide skilled nursing care to home health patients based on individualized care plans. Responsibilities include wound care, IV administration, lab draws, PICC and drain care, medication administration, and monitoring vital signs. Collaborate with PT/OT/ST, maintain infection control protocols, and ensure accurate documentation of all care provided. Report abnormalities and coordinate supply and medication needs. Monitor incidents, ensure proper reporting, and oversee safe use of medical equipment.
Requirements:
Active LPN license (TN or compact state) Strong clinical skills in wound care, IV therapy, and home health Proficiency with HCHB software #
