Sunbelt Staffing is currently seeking a dedicated and experienced Home Health LPN for a rewarding 13-week contract position in Nashville, TN. The ideal candidate will have at least 1 year of home health experience and hold a current American Heart Association BLS certification.

Contract Duration:
13 weeks
Contracted Hours:
40 hours per week
Schedule:
Monday to Friday, 8:00 am – 5:00 pm
On-call:
Required at least 1 weekend per month
Requirements:
Current LPN licensure in the state yourapplying for, or compact Minimum of 1 year of experience in home health American Heart Association BLS certification
Responsibilities:
Conducting comprehensive patient assessments and reassessments to determine ongoing care needs Collaborating with physicians and other healthcare professionals to develop and implement individualized care plans Performing skilled nursing procedures, including wound care, IV administration, and medication management Monitoring and documenting patient progress, ensuring accurate and timely record-keeping Educating patients and their families on proper self-care techniques and health maintenance strategies Coordinating with other members of the healthcare team to ensure seamless and holistic patient care Participating in interdisciplinary case conferences to discuss patient care plans and make necessary adjustments Responding promptly to patient emergencies, providing immediate care and coordinating with emergency services as needed Adhering to all relevant policies, procedures, and regulations to maintain compliance with healthcare standards Engaging in continuous learning and professional development to stay current with evolving healthcare practices and technologies
