Job Summary:
We are seeking a dedicated Hospice LPN to join our compassionate team. In this role, you will provide skilled nursing care to terminally ill patients in their place of residence, collaborating with the interdisciplinary team, families, and other healthcare professionals. You will follow established care plans under the direction of a Registered Nurse, ensuring the delivery of high-quality hospice care in accordance with state law and the Nurse Practice Act.
Key Responsibilities:
Patient Care :
Administer nursing care as per the established plan of care, ensuring compliance with the Nurse Practice Act and state laws.
Skilled Nursing Services :
Provide direct bedside nursing care including assessment, evaluation, procedures, administration of treatments and medications.
Patient/Families Coordination :
Evaluate patient and family needs, assisting with the preparation and execution of individualized care plans.
Infection Control :
Practice infection control measures and follow standard precautions.
Collaboration :
Work with the Registered Nurse and interdisciplinary team to revise care plans and adjust goals as patient needs change.
Bereavement Support :
Participate in providing bereavement support to families, including closure contacts and related activities.
On-Call Support :
Participate in on-call visits after triage by the Registered Nurse and complete routine visits as needed.
Documentation :
Accurately document clinical notes in accordance with agency policies and procedures.
Qualifications:
Required:
Minimum of two years LPN/LVN experience in oncology, long-term care, hospice, or home health. Active state nursing license . Current BLS certification .
