Job Description
As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.
Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.Performs plan of care interventions, including specimen collection and wound care.
May administer medication.
Documents patient care given.
Assists RN with development of plan of care.
Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
Monitors, documents, and communicates patient’s status and reactions to interventions with RN, physician, patient, and families.
Works as an advocate for the physical and emotional well-being of the patient. Perform other duties as assigned. Attends and participates in staff meetings, in-service, projects and committees as assigned. Adheres to and supports policies and procedures of the Highpoint Health with Ascension Saint Thomas. Works scheduled shifts including overtime, when necessary. Accepts all call schedule as directed. Maintains a neat and professional appearance in compliance to the existing dress code. Compliance to Highpoint Health with Ascension Saint Thomas Standards of Behavior is required. Compliance to Dress Code Standards. While performing this job, occupational exposure to bloodborne pathogens is present for all employees. Reports to the Director of Nursing, Skilled Care. Job RequirementsMinimum EducationMust be a graduate of a Practical Nursing Program.

Must be able to work in a stressful environment and take appropriate action. CertificationsAHA Basic Life Support (BLS) within 30 days of hire. LicensesLPN License for the State of Tennessee.
