Overview of LPN Responsibilities:

• Provide high-quality nursing care to residents in long-term care, nursing homes, skilled nursing, and rehabilitation settings
• Administer medications and treatments per physician orders and facility protocols
• Observe, monitor, and document resident conditions and report changes promptly
• Collaborate with RN’s and CNA’s to implement individualized care plans
• Ensure compliance with all state and federal guidelines for

LPN’s Requirements:

• Active LPN license in the State of Wisconsin
• Current BLS certification
• Minimum 1 year of recent clinical experience required
• Compassionate, dependable, and ready to be On Point Pay Rates (Based on role and shift type): Licensed Practical Nurse (LPN) $40
• 55/hr + Rates vary & can go higher
• based on shift urgency, facility location, and day/time.

• Rehabilitation Shift Types:

• 4-hour AM, PM, and NOC shifts
• 8-hour AM, PM, and NOC shifts
