Providesprescribed medical treatment and personal care and services topersons with disabilities in residential homes and/or vocationalcenters at a single business location. Relies on knowledge,experience, and judgment to accomplish tasks in combination withclinical supervision from the registered nurse and direction fromthe Registered Nurse and/or Administrator. Requires graduation froman accredited licensed nurse program and current LPN license withno restrictions to practice nursing.

SUPERVISORY RESPONSIBILITIES

This position has nosupervisory responsibilities.

Job Responsibilities:

Provide holisticnursing care to individuals with intellectual and developmentaldisabilitiesObserve, assist with gathering health related data, andreport the health needs or the people we support to the registerednurseCoordinate preventative and routine medical care for peoplesupportedDocument nursing care providedAssist physicians withon-site medical clinics where applicableProvide health relatedtrainings to unlicensed personnel according to stateregulationsConduct medication pass observations in the home toensure staff competency according to state regulationsConductmedication closest assessmentsPerform quality assurance checks andobservations to ensure unlicensed personnel are performing healthrelated tasks as assigned according to state regulationsObserve forand report any abnormal health related findings to thephysicianCommunicate with physicians, pharmacies, medicalsuppliers, and the Interdisciplinary TeamAdvocate for the peoplesupported and assist with developing community supports to promoteoptimal health for the people supported#

HPNURSEMINIMUMQUALIFICATIONS

Graduation from an accredited licensed nurseprogram.

Current LPN license with no restrictions to practicenursing.

Current CPR Certification or eligibility to certify forCPR.Possess credentials required by Federal or State lawMust beable to communicate effectively with Interdisciplinary TeamMembers, as well as the people supported and their families.

Must beable to work independently, be organized, and possess the abilityto prioritize work tasks.

Must be at least 18 years of age

PHYSICALDEMANDS AND WORK ENVIRONMENT

Regularly required to lift 20 lbs.

Mustbe able to lift a minimum of 40 lbs.

Must be able to pull a minimumof 25 lbs.

Must be able to squat, kneel, crawl, crouch, climb, andstoop.

Must be able to regularly use hands to finger, handle, orfeel objects, tools, or controls.

Required to regularly stand andwalk.

Must be able to demonstrate proficiency in CPR from the floorlevel requiring to work on hands, knees, bending, standing andlifting.

Vision requirements include close vision, distance vision,and peripheral vision.

Must be able to talk and hear.
