Position Summary:

Oversee and promote the residents’ health and wellness through regular visits with each resident, resident assessment, and ongoing communication with the resident care team as well as participation in the Interdisciplinary Team evaluation. Provide hands-on supervision of caregiver staff, ensure provision of quality resident care, and uphold residence philosophy and resident well-being. Provide direct assistance with activities of daily living and support to residents. Provides medication administration of scheduled medications to residents in accordance with state regulations policies and procedures, and ensure the residents’ rights to safe, prompt, and confidential treatment.

Primary Responsibilities:

Resident Health and Wellness 1. Meet with each resident on a regular basis to provide health and wellness- checks and answers any health-related questions. 2. Review all resident information and complete an assessment of the resident at move-in, when changes in the resident’s condition occur, and as required by residence policy and state regulation. 3. Review the resident’s medical report prior to move-in to ensure that the residence can provide for the resident’s individual needs and regulatory requirements have been fulfilled. 4. Order medications, check in medications, store medications away, and oversee the resident’s medication schedule for residents receiving medication administration services. 5. Maintain a neat, clean, organized medication cabinet, and restock the cabinet as needed. 6. Ensure proper documentation for medication assistance/administration • Follow the six “rights” of medication administration (right resident, right drug, right dosage, right time, right route, and right record/documentation) each and every time medications are administrated. Check each of the six “rights” of medication administration three different times: _When the medication is taken out of the medication drawer. _Immediately before assisting with administration of the medication. _Immediately after assisting with administration of the medication. 7. Assist the Community Director to develop and maintain an up-to-date individual service plan for each resident. 8. Maintain ongoing communication with the resident, resident’s family and physician regarding the resident’s medical needs. 9. Coordinate additional health services the resident may need. 10. Assist in coordinating resident transfer to and from a hospital or another facility. 11. Assist with counseling residents and families. 12. Be on call for medical questions and resident emergencies as scheduled. 13. Assist in coordinating residents’ dietary needs with the Food Service Director. 14. Review changes in residents’ condition with the Community Director, Nursing Director and resident’s physician/family member or POA. 15. Assess residents and monitor their health. 16. Ensure that all infection control guidelines are met. 17. Maintain residents’ care files/binder. 18. Supervise the caregiver staff on duty in providing quality care and meeting the needs of the residents in accordance with residence philosophy and policies. 19. Answer and respond immediately to resident calls. 20. Assist residents with their activities of daily living. 21. Assist with resident activities. 22. Assist with serving meals and supervise caregiver staff in the dining room. 23. Ensure that dining room is cleaned after each meal service. 24. Assist with preparing and cook of food in accordance with nutritional guidelines, regulations, and resident’s individual prescribed diet and food preferences as needed. 25. Assist with residents’ personal laundry as needed. 26. Assist with cleaning and tidying residents’ rooms/apartments as needed. 27. Review daily any and all communication tools used in providing resident care. 28. Communicate residents’ needs with caregiver staff at change of shift and as needed. 29. Carry out other duties as assigned by Community Director including on-call responsibilities.
