Requirements:

Responsibilities:

Perform selected nursing procedures withinthe scope of LPN practiceProvide patient and family teachingaccording to plan outlined by the RNMonitor patient’s vital signsand make specific measurements and observationsMonitor and recordintake and output. Report significant changes immediatelyObserve,assess and record patient’s physical and mental condition andreports any changesAdminister medications as instructed and asordered by the physician and instructed by the RN and document thepatient’s file accordinglyAttend and participate inmulti-disciplinary patient case conferences, staff meetings andin-service education programs as assigned

Qualifications:

LicensedPractical Nurse / LPN license in good standingPrevious privateduty, home health, hospice, hospital or skilled nursing experiencepreferredActive CPR certificationFamiliarity with electronic healthrecordsStrong problem solving and critical thinking skillsReliabletransportation, valid driver’s license and current automobileinsuranceAbout our
