We are looking for a dedicated Home Health LPNlike you to be a part of our team. Job Summary and QualificationsAdministers quality, skilled nursing care to patients in the home.

Work in accordance with physician orders and the Plan of Care underthe direction and supervision of a Registered Nurse and/orDirector, Professional Services. Performs all functions in aprofessional and ethical manner and collaborates with other membersof the health care team to ensure the provision of quality patientcare.

What you will do in this role: Under the direction of a RN, provides patient care andidentifies the physical, psychosocial, and environmental needs ofpatients. Verifies Plan of Care prior to each visit and provides careaccording to the physician’s orders, assessment data, establishedstandards and guidelines and documents accordingly. Incorporatesshort and long term goals as established by the Plan of Care intonursing care, as evidenced by documentation in the nursingnotes. Performs an ongoing assessment during each visit and documentsdata in patient medical Communicates significant findings,problems, or changes in the patient’s condition to the supervisingRN and/or Director of Professional Services and the physician anddocuments all findings, communications and appropriateinterventions. Documents nursing interventions including patientresponse. Assesses patient/caregiver’s knowledge of disease process,treatment regimen and/or medication administration. Provides basicand specialized teaching as ordered in the Plan of Care and basedon the patient’s needs. Identifies and recommends other disciplinesneeded for quality care of the patient.

What qualifications youwill need:
