Position Scope:

The Health andWellness Director is responsible for directing the Health andWellness program of the community. Ensures that Park AvenueLifestyle standards and state requirements for resident care aremet. Directs staff, ensures regulatory compliance and isresponsible for the management of the clinical budget.

RESPONSIBILITIES

Supports the mission, vision and goals of theorganization, upholding and promoting company culture and vision.

Directly oversee the delivery of care to the residents of thecommunity to ensure that emotional, physical, psychological andsafety needs are met through all program services and activities.

Coordinate, along with the Executive Director, family/resident careconferences and serve as a direct liaison between residents,families, and staff. Worth with the residents, families, and otherstaff to ensure that residents are at appropriate levels of care.

Manage labor costs, supplies and other line items in the clinicalbudget. Complete weekly, monthly reports as required. Monitordocumentation systems to ensure that all regulatory timeframes forreview of assessments and service plans are met and that residentneeds are met. Write and review residents’ assessments/evaluationsand care/service-plans with appropriate staff. Assess/evaluatepotential residents for admission into the community and todetermine level of care. This may involve assessing/evaluatingpotential residents in their home or another healthcare setting;must be able to drive to location and perform assessment/evaluationprofessionally and timely. Schedule and conduct new residentorientation for residents upon move into the community. Provide,plan, schedule and monitor staff training programs and in-servicesfor staff including statutory or regulator required training.

Attend all required training, in-service, and staff meetings.

Manage and coordinate the work of all nursing department/clinicalstaff of the community, communicating pertinent resident health andsafety information. This includes overseeing and/or completingstaff schedules at least monthly and daily assignments. Strive tomaintain a safe working environment through the prevention ofaccidents, the preservation of equipment, and the achievement ofsafe working practices. Ensure compliance with all state regulatoryrequirements including mandatory reporting to the State. Ensurethat all concern and incident reports are completed, filed and thatfollow-up in conducted appropriately. Assist and/or perform in mocksurveys. Maintain a positive and professional demeanor towardresidents, visitors, families, and co-workers while adhering to allpolicies, standards, and procedures of the Company. Visitsresidents in hospital/rehab. as assigned. Organizes andcommunicates with leadership, residents, associates and familiesregarding annual flu clinics, TB testing of associates/residents.

Participates in community Wellness Programs as assigned.

Participates in MOD weekend and holiday coverage, as assigned.

Member of the backup sales team. Participates in on-call scheduleas assigned. Counsels’ residents in identifying and resolvingsocial or other problems. Requires 24/7 on-call clinical support.

Attends training courses at community and maintains up to datemonthly, quarterly, annual training through designated LearningManagement System (LMS). Other duties as assigned, and which relateto the success of the community and the care, comfort, andhappiness of our residents. CB Requirements

QUALIFICATIONS

Education as required to obtain state nursing license RN preferred.

LPN considered with strong clinical leadership experience.

Preferred a minimum of 3 years clinical leadership experience.

Working knowledge of basic technology and electronic documentation.

Ability to demonstrate appropriate judgment skills and independentclinical decisions in routine and emergency resident care matters.

High level of interpersonal skills to handle sensitive andconfidential situations and documentation. Ability to workevenings, weekends, and flexible hours when necessary to meet theneeds of the residents. Excellent verbal, oral, and writtencommunication skills Valid driver’s license: ability to travel toconduct resident assessments and required trainings forprofessional development. Must have current RN licensure in goodstanding.
