SHIFT- MONDAY- SUNDAY

(FLEXIBLE DAYS) 7P-7A We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients in various settings, including clinics and inpatient facilities. This role requires a strong understanding of internal medicine, dementia care, and infection control practices. The ideal candidate will demonstrate excellent clinical skills, effective communication, and a commitment to patient-centered care. Responsibilities Administer medications and treatments as prescribed by physicians. Monitor and record vital signs, ensuring accurate documentation of patient health. Assist in the management of patients with chronic conditions, including dementia care. Provide health coaching and education to patients and their families regarding treatment plans. Perform procedures such as catheterization and airway management as needed. Collaborate with healthcare teams to develop and implement patient care plans. Maintain a clean and safe environment while adhering to infection control protocols. Participate in the preparation of patients for examinations or procedures in the clinic or operating room (OR). Support the dialysis process for patients requiring renal care. Qualifications Valid LPN license in the state of practice. Experience in internal medicine or related fields is preferred. Knowledge of dementia care practices is highly desirable. Proficiency in performing vital signs assessments and basic nursing procedures. Familiarity with health coaching principles to support patient education. Ability to perform catheterization and airway management competently. Understanding of infection control standards within healthcare settings. Strong interpersonal skills with the ability to communicate effectively with patients, families, and team members.
