Job Summary:

We are seeking a highly motivated and experienced Licensed Practical Nurse (LPN) to join our team as a Care Coordinator. This hybrid role involves both in-office and remote work, focusing on care coordination, chronic care management (CCM), and the development and implementation of individualized care plans. The ideal candidate will have a passion for patient care, excellent communication skills, and the ability to work effectively in a team-based environment.

KEY RESPONSIBILITIES
Care Coordination:

Coordinate patient care activities across various healthcare settings to ensure continuity and quality of care. Facilitate communication between patients, families, and healthcare providers. Schedule and participate in interdisciplinary team meetings to discuss patient care plans and progress.

Care Plan Development:

Collaborate with healthcare providers to develop and implement individualized care plans based on patient needs and preferences. Monitor and update care plans regularly to reflect changes in patient health status and goals. Chronic Care Management (CCM): Provide chronic care management services to patients with multiple chronic conditions. Conduct regular follow-up calls and visits to assess patient progress and adherence to care plans. Educate patients and families on disease management, medication adherence, and healthy lifestyle choices.

Training and Education:

Participate in ongoing training and professional development to stay current with best practices in care coordination and chronic care management. Provide training and support to other healthcare team members on care coordination processes and protocols.

Documentation and Reporting:

Maintain accurate and timely documentation of all patient interactions and care coordination activities in the electronic health record (EHR) system. Generate and review reports to track patient outcomes and identify areas for improvement. .

POSITION REQUIREMENTS
Education :

Current LPN license in the state of TN.

Experience :

Minimum of 1-2 years of experience in a clinical setting, preferably in care coordination or chronic care management.

Skills and Abilities :

Strong understanding of care coordination principles and practices. Excellent communication, organizational, and interpersonal skills. Proficiency in using electronic health records (EHR) and other healthcare technology. Ability to work independently and as part of a multidisciplinary team. Commitment to providing high-quality, patient-centered care.

Licenses or Certifications :
