SUMMARY OF POSITION
The position of (LPN), MDS Coordinator is responsible for the completion of the RAI Assessment and the certification, accuracy, and completeness of the Minimum Data Set (MDS). The position exists to ensure MDS and care planning objectives are achieved.
PRIMARY DUTIES & RESPONSIBILITIES
Attend seminars or any other training to increase knowledge and competence; ensure good cohesive interactions and productive sessions within the interdisciplinary team. A comprehensive assessment of a resident’s needs and capabilities based on uniform data used as an instrument by the state. The assessment should describe the capability of the residents to perform daily life functions and their significant impairments in functional capacity. The assessments will be conducted and coordinated by the MDS Coordinator who will sign and certify the completion and accuracy of these assessments. Findings of the interdisciplinary group will be formulated together during the assessment and care planning process. Each discipline will sign and attest to the accuracy and agreement of decisions made regarding care. Electronically submit MDS to the State as required. Coordinate with the staff and interdisciplinary team in gathering and analyzing information in order to formulate the individual plan of care. Develop a comprehensive care plan for each resident including measurable objectives and timetables to meet medical and psychosocial needs that are identified in the comprehensive assessment. Perform as needed as floor nurse to carry out nursing procedures and administer medication per physician’s plan of care. License/Qualifications State of TN or Multi-State License as a Licensed Practical Nurse (LPN) or Registered Nurse (RN). Three (3) years of experience working with MDS in a SNF preferred.
