The PCMH Coordinator will assist with the management of the computerized data repository, including generating population health data reports, utilizing data for population health management, and addressing gaps in service and care.

Duties and Responsibilities:
  • Identifies the needs of patients through patient engagement and relationship building and provides appropriate support to respond more effectively to those needs, utilizing both clinical and community resources.
  • Assesses barriers when patients are not meeting treatment goals, following treatment care plan, or keeping scheduled appointments.
  • Participates in PCMH team meetings and quality improvement initiatives.
  • Supports patient self-management of disease and behavior goals and modification interventions.
  • Coordinates continuity of medical care with patients and families following hospital discharge and ED visits.
  • Works collaboratively with members of the care team to identify patients who need disease management and educate them on the importance of managing their conditions to help close gaps in care.
  • Utilizes a computerized disease registry to manage high-risk patient care, including management of patients with chronic conditions (e.g., diabetes, asthma, hypertension, coronary artery disease, etc.), multiple comorbidities, or high-risk for hospital readmission or ED visits.
  • Identifies patients needing preventive screenings and reviews the resources available to encourage them to obtain preventive services.
  • Facilitates patient medication management based upon standing orders and protocols.
  • Participates on team(s) for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the PCMH initiatives.
  • Evaluates clinical care, utilization of resources, and work in collaboration with the Quality Director in the development of new clinical tools, forms, and procedures.
  • Coordinates training activities related to PCMH data collection, patient tracking, policy and procedure development, and workflow changes.
  • Works with the Quality Director to develop, collect and prepare PCMH presentations and educational materials.
REQUIREMENTS

Licensed to practice as a nurse (LPN) in Tennessee or other licensed medical professionals who possess the appropriate clinical skills Two (2) years’ experience working with chronic disease management and/or case management in an outpatient or ambulatory care setting Proficient in Microsoft Office, especially Word, Excel and PowerPoint Ability to prioritize and manage multiple tasks concurrently Excellent written and oral communication skills We offer competitive pay and benefits for all full-time employees.
