    (LPN)

  • Definition:
  • The Licensed Practical Nurse (LPN) provides skilled nursing services under the direction and supervision of the Registered Nurse.
  • Line of Authority:
  • Registered Nurse, Clinical Manager; Administrator/Director of Services; Regional Administrator; AVP, HomeCare Clinical Operations, Vice President, HomeCare
  • Qualifications:
  • 1.

Unencumbered, current Licensed Practical Nurse license in the state where the HomeCare agency is located or compact state if applicable 2. Graduate of a state approved LPN program 3. Minimum one (1) year clinical experience as an LPN a. Individuals with less than one full year’s experience as an LPN will be considered. i. If hired, they will be mentored and provided additional oversight through the end of that 1-year period. 4. Experience in home health care is desirable.

  • Performance Requirements:

Ability to read, write, learn, absorb, and follow written orders and keep records in prescribed manner. 2. Ability to be reaching, bending, squatting, and twisting numerous times daily. 3. Ability to lift 70-80 pounds on occasional basis. 4. Ability to see and hear adequately in order to respond to auditory and visual requests from patients. 6. Ability to speak in clear, concise voice in order to communicate adequately. 7. Ability to carry out fine motor skills with manual dexterity. 8. Able to perform routine nursing procedures. 9. Mental acuity high enough to adequately perform job requirements. 10. Must have reliable personal transportation and be able to drive in inclement weather and variable road conditions.

  • Specific Responsibilities:

Under the supervision of a Registered Nurse, provides skilled nursing services as ordered on the plan of care and according to policy/procedures and state practice acts. LPN_ R 7/08, 11/13, 10/16, 9/2021_ page 1 of 2 2. Instructs the patient/caregiver in various aspects of patient care and disease management. Documents the patient/caregiver comprehension of and compliance with teaching. 3. Observes patient response to care/teaching and makes recommendations regarding revision of plan for patient care to the Registered Nurse. 4 Provides and documents skilled nursing care according to the established plan of care, policies/procedures, and standards of care. 5. Provides care to the patient including administration of treatments and medications within the scope of practice authorized by the license issued by the state for a licensed practical nurse. 6. Records and reports to the Registered Nurse symptoms, vital signs and other observations that may be indicative of care plan reactions and changes. 7. Follows and teaches infection prevention and control and safety policies and procedures. 8. Participates in on-going care coordination and maintains positive relationships with referral sources, physicians/other clinicians, patients/families, and other team members. 9. Participates in the after-hours on-call rotation with other nurses on a rotating basis and under the direction of a Registered Nurse. 10. Continually strives to develop clinical skills and knowledge through participation in inservice activities, staff development activities and continuing learning. 11. Participates in and supports Quality Assurance Performance Improvement (QAPI) activities including serving on QAPI Steering Committee / Performance Improvement project (PIP) groups as assigned. 12. Performs other duties as assigned by the RN / Clinical Manager.
