Essential Duties and Responsibilities:

Utilizes the nursing process to plan, implement, and evaluatepatient/client care and/or contracted services Demonstrates on eachassignment the provision of skilled nursing care rendered inaccordance with prescribed orders Identifies and documents changesin patient’s both physiologic and psychosocial condition NotifiesDirector/Supervisor if applicable on the following; o Physiciannotification to obtain necessary orders for intervention(s) perstate regulations o Changes in patient/client condition Adheres toMaxim’s policies and procedures and applicable protocols Identifiesand prioritizes actions based upon patient/client care requirementsAssures patient safety at all times and recipient of servicesprovided Verifies that documentation reflects patient/clientcollaboration, participation in plan of care, expectedpatient/client outcomes, and selected nursing interventionsPerforms interventions, treatments, and medication administration,according to identified priorities, plan of care, facility policiesand patient/client care outcome standards as applicable. Monitorsand documents effectiveness of interventions, treatments, andmedications Evaluates patient/client and provides a safe,comfortable and therapeutic patient/client environment Monitors thesafety and effectiveness of the environment and equipment includinguniversal precautions Participates in activities to facilitate goodinterpersonal communications Communicates observations andsignificant changes in patient/client status to the Director ofClinical Services and/or Clinical Supervisor /Case Manager orlicensed designee Documents all physiciancollaboration/coordination of care in the medical record andcommunicates this information as needed Attends case conferences,team meetings, education in-services as required Supportsprofessional practice by observing the following; o Nurse PracticeAct o Standards of Nursing Practice o Standards of Patient Care oStandards of Accrediting Body (ACHC, Joint Commission, etc.) oState and Local Laws o Patient Bill of Rights o Maxim Healthcare,Nursing, and Client’s Hospital Policies o Performs other duties asassigned

Minimum Requirements:

Graduate of an accredited school ofnursing License Required 3.0 GPA preferred Complies with acceptedprofessional standards and practice Current CPR PPD or chest X rayif applicable Successful completion of Maxim Homecare Novice NurseProgram orientation and training Proficiency in the Englishlanguage required Excellent communication skills Technologicalproficiency, including mobile devices for documentation
