JOB SUMMARY

The Lead Medical Assistant/Clinical Care Coordinator (CCC) provides hands- on coordination of clinical care activities and as the clinical lead, provides the knowledge and expertise necessary to provide effective clinical support for clinical providers, medical assistants, translator and other support team members. In addition, he or she serves as the clinical lead for care coordination and care management services for patients. Core responsibilities include personalized disease management services; coordinating continuity of patient care with external healthcare organizations and facilities; patient education and health behavior coaching; patient follow-up and tracking; assisting with quality improvement audits, drill downs and tests for improvement; administering immunizations and other medications; providing additional clinical support for high-risk patients; and assisting with direct patient care when needed. Bilingual English/Spanish required.

SPECIFIC TASKS AND RESPONSIBILITIES

The Clinical Care Coordinator actively participates in the continuous quality improvement of GHMS’ delivery of services and the continuous innovative redesign of those services. Actively participates in quality improvement initiatives for clinical support staff members at GHMS, including the use of the Plan, Do, Study, Act cycle, the Care Model and other QI methodologies. Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits. Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures. Has working knowledge of clinic equipment and procedures, i.e. Blood Pressure machine, EKG machine, Audiometer, A1C machine, Spirometry, Nebulizers, Accucheck, Strep and Flu tests, etc. Prepares and administers immunizations and medications as directed by the provider according to the licensure of the physician and the State of Georgia. Assists in maintaining an inventory of supplies and equipment. The CCC may provide direct patient care as Medical Assistant when needed. Performs general office duties, such as answering phone calls from patients, completing required documentation for patient care, or other office tasks as requested by provider(s) or supervisor(s). Works with front office staff and scheduling staff to ensure acutely ill patients are seen as needed. The CCC performs other duties as assigned by the physicians, mid-levels or supervisor(s).

QUALIFICATIONS

Graduation from an accredited school of nursing, LPN, or graduate of technical school in Medical Assisting or comparable experience in a medical office setting. 2-5 years’ experience in a clinical setting. Experience with electronic medical records. Proficient computer skills, including Microsoft Office and EMR (eClinical Works a plus). Self-disciplined, energetic, passionate, innovative. A team player that can follow a system and protocol to achieve a common goal. Highly organized and well-developed oral and written communication skills. Demonstrates sound judgment, decision-making and problem-solving skills.
