Job Summary The Clinical Care Partner MA is responsible for coordinating pre visit patient care following established standards and practices of population health management. Monitors disease registry for patient scheduled appointments, procedures, tests and referrals while communicating high risk patient needs to registered nurse care coordinator. Relies on guidelines to accomplish tasks. Relies on some experience to complete a job. Works under close supervision. Minimum Requirements Experience

Coordination of Care Reviews evidenced based care measures for population of patients, assists providers with organizing pre-visit patient care need to ensure that the care needs are outlined and established prior to visit to optimize the patients time with the provider during visit. Prepares for clinic interdisciplinary team meetings by confirm previous orders have been completed and resulted, review inpatient admissions and emergency room visits, review anticipated annual screenings, prepare agenda for meetings, print patient care summary for providers as needed, identify high risk patients and communicate with team for follow up. Work within the data warehouse and disease registry to identify gaps in laboratory values, medications and appointments, work with patients via telephone or face to face to encourage and support compliance. Observes, records and reports (to physician) patient’s condition and reaction to drugs and/or treatments. Instructs patient/family regarding diagnostic procedures, medications, nutrition and health/wellness issues. Observes and adheres to quality standards established by the department. Participates in continuous quality improvement programs in order to foster the provision of quality health care services by the department and hospital and ensures that all services are provided with compassion, understanding, and respect for others. 2.
