We believe everyone should have confidence in navigating their healthcare. We are looking for a qualified Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) to serve our patient population in the navigation, prevention and management of their health through continuous care programs like Remote Patient Monitoring and Chronic Care Management. Our team works fervently to anticipate the needs of our patients and connect on a personal level. We exist to coach people to their best health! Responsibilities Manage a roster of patients who are enrolled in the Chronic Care Management (CCM) and Remote Patient Monitoring program (RPM). Act as a direct liaison between the patient and the office/provider Scheduling and confirming appointments in office for the patient. Have a full understanding of each patient’s individual care plan and making sure they are following it. Record patient data daily and alert healthcare providers of abnormal reports. Qualifications At least 1 year of clinical work experience as a Medical Assistant or Registered Medical Assistant High level of professionalism Excellent communication skills (oral and written) Variety of Electronic Medical Record (EMR) experience Proficient knowledge, skill, and interest in basic computer skills Proficient in Microsoft Words and Excel Proficient in problem solving and ability to multi-task Medical Assistant Course Completion Diploma/ Certification Excellent teamwork skills Supportive and positive work environment Join us in making a positive impact on our community through effective engagement and outreach initiatives!
