Coordinate with physicians with creating and updating Care Plans for diagnoses specific as needed for patients in the program. Collaborate with healthcare professionals to facilitate effective communication regarding patient needs if revealed during CCM monthly communications. Perform patient care in-clinic when needed/requested by current medical assistants Skills Proficient in medical terminology and current medications. Ability to provide excellent patient service while managing multiple tasks efficiently; proficient typing skills. Strong organizational skills and attention to detail in managing patient information within the CCM Program and patient chart.. Effective communication skills for interacting with patients and family members that are encountered during CCM contact. Medical assisting/triaging/cross training to cover patients when needed .
