JOB SUMMARY

Supports the Patient Care Coordinator staff by managing correspondence and maintaining files for HHA (Home Health Aide), DME (Durable Medical Equipment), Supplies, and Transportation contractual agencies. Coordinates the ordering and delivery of DME and supplies to patient homes. Supports the Provider Relations and Community Field Staff while assigned to specific Interdisciplinary teams.

RESPONSIBILITIES

1. Maintains all DME/Supply/HHA/Transportation staff agency records in binder or electronic file. Orders and discontinues services as directed by the Patient Care Coordinator. 2. Maintains records of HHA hours, via Allscripts Scheduler Module. Performs and records daily attendance check of Hospice HHAs. Performs daily verification of HHA visits and activities in the Allscripts Scheduler Module. Forwards HHA notes to Patient Care Coordinators for review. 3. Maintains vendor HHA profiles. 4. Handles all typing, copying, filing and mailing necessary to support the Patient Care Coordinators. 5. Orders medical supplies as directed by the Hospice Nurse and/or Patient Care Coordinator. Maintains order records and checks invoices received for accuracy. 6. Assists the Patient Care Coordinator in preparing reports and special projects as requested. 7. Reviews daily on-call report. 8. Arranges patient transport and maintains all records. 9. Notifies assigned vendor via fax or e-mail of DME and/or Hospice HHA cancellation upon patient discharge, transfer to IPU, and/or death. Arranges timely pick up of DME. 10. Creates, maintains, and updates charts for all newly admitted patients. Maintains the open chart by filing all clinical documentation daily as it is received. 11. Processes all forms that need to be mailed/faxed and creates daily order run for: a. Initial Plan of Care/Certification of Terminal Illness/Recertification b. Supplemental Physician Orders c. Discharge notification and service evaluation forms to the patient’s Attending Physicians. 12. Files and maintains all medical records in designated area according to agency policy and procedures. 13. Other duties as assigned by the Team Director, or designee.

QUALIFICATIONS

1. High School Diploma or GED required 2. Previous office experience and/or business education. 3. Demonstrates excellent verbal and written communication skills. 4. Demonstrates organizational skills and knowledge of medical terminology. 5. Knowledge of Windows ® computer operating system required; basic working knowledge of MS Office suite and computer generated clinical records experience preferred 6. At least one-year previous clerical experience, preferably in a medical setting. Certification in Medical Record Information Technology preferred. 7. Demonstrates commitment to hospice philosophy of care. 8. Demonstrates an ability to work closely with a variety of people while effectively using problem-solving skills. 9. Professional and pleasing telephone etiquette a must. 10. Bilingual skills, English/Spanish, English/Cantonese and/or Mandarin preferred but not required. 11. Covid-19 Vaccinated.
