Essential Functions ℜsponsibilitiesThe physician serves as medical director forskilled nursing facilities (SNF) and participates in administrativeand quality initiatives for the facilities he or she is responsiblefor;Medical directorship functions include attending andparticipating in monthly quality assurance and process improvementmeetings; participating in quality improvement initiatives;providing guidance to facility staff; reviewing and revising (ifnecessary) facility’s clinical guidelines; insuring compliance withstate and federal regulations, training facility staff; andsupervising facility clinical staff.

In addition to the medicaldirector role, the physician also serves as the attending physicianfor the residents residing at each facility;The physiciancollaborates with Nurse Practitioners and Physician Assistants tomanage day to day resident care at each facility. Typically, eachfacility will have a full-time PA or NP assigned to the facilitydepending on the acuity and volume of the patientpopulation;Physician will provide high quality, patient-centered,comprehensive primary care services by rounding on patientsresiding in SNFs;Physician is responsible for diagnosing andtreating patients with acute illness, such as bacterial and viralinfections, as well as chronic diseases and conditions, such ashypertension, heart disease, or diabetesProvider documentation iscompleted in Theoria’s proprietary electronic health record (EHR)system, ChartEasyPointClickCare, MatrixCare, and AmericanHealthTech are the most commonly used facility EHRsystemsParticipating in Theoria’s Chronic Care Management,Transitional Care, RPM and other medical quality initiatives andprogramsPhysician must be able to supervise collaborating nursepractitioners and physicians assistantsPhysician must be able toutilize electronic/online medical tools such as EHR systems andstate/county medical systems utilized for Death Certificates andadministrative functions.

ShiftStructureShifts are flexible depending on physicians’schedule;Typically, medical directors work a minimum of 2-3days/week at each facility they oversee;Rounding times are flexiblewith no set start and end time.
