You will be an executive in our large integratedhealthcare organization, sharing your passion for whole person careas a Primary Care Provider, providing care in English and Spanish,and overseeing primary care teams leveraging your background inprimary care and integrated healthcare.

You will support ourteam-based primary care model that includes case management,nurses, and, with our recent merger, expanding our integration inwhole person care with behavioral health professionals. Yourpassion for quality outcomes within the population health frameworkwill guide your assessment from an evidence-based diagnosisperspective.

At CHFW, you will manage value-based payments (VBP) andthe VBP system, ensuring insurance payers receive our outcomesbased on quality and deliverables. You will understand therequirements and know what is needed to achieve the requiredoutcomes.

In our collaborative environment, you will work with otherexecutives and the senior leadership team to maximize outcomes forour patients. You will provide our Board of Directors with riskreporting through written reports and in-person presentations. Inyour role, you will be visible and present within the community,including with state-level representatives.

Minimum Requirements:
