Position Overview:
Our Physicians have flexibility to devote asmuch time as needed for the clinical and social needs of ourparticipants and their caregivers to provide individualized qualitycare by a team of geriatric care professionals who work togetherwith participants and caregivers to address each individual’sspecific needs. This team of professionals is an essentialcomponent of the Summit ElderCare PACE program. The team uses acollaborative approach to care planning and is called theInterdisciplinary Team (IDT). The IDT is comprised of Providers(MD/DO and NP/PA), Nurses, Social Workers, Physical andOccupational Therapists, Dieticians, Health Aides, Recreationstaff, Home Care and Transportation Coordinators and SiteAdministrator. This approach to care is the gold standard forcomplex and frail older adults who wish to avoid nursing homeplacement.
Brief summary of purpose:
The primary role is to providehigh quality, cost effective medical care to program participantsin accordance with the program mission and participant goalResponsibilities Highlights of Summit ElderCare

- Specialized dementia and geriatric carePrimary Job Responsibilities
- Support the intake and enrollmentprocess by meeting with participants and caregivers to representthe program and conduct initial assessments.
- Conduct biannualparticipant assessments.
- Assist with the development andmaintenance of participant care plans.
- Participate in theinterdisciplinary team (IDT) process.
- Provide routine, preventiveand urgent care to participants.
- Promote strong colloborativerelationships between participants, their caregivers and other PACEstaff.
- Provide and arrange for specialty care consultations insupport of participant care plans. Incorporate and coordinatespecialist reports and recommendations into the medical record,assessments and care planning.
- Complete all requireddocumentation in the participant chart and other forms as directed
- Participate in after hours on call arrangements as determined bythe PACE Medical Director.
- Provide care and assessments ofparticipants in the most appropriate setting including the PACEclinic, inpatient facilites or participants’ homes.
- Participatein committees and work teams as requested by the Medical Director
- Work in partnership with the PACE transitional care coordinatorto support achievement of quality and utilization goals.
- Developeffective collaborative relationships with community physicians andother providers.
- Perform participant chart audits as requested bythe PACE Medical Director.
- Document complete diagnostic andprocedure codes in the participant chart and encounter forms tosupport care plans and accurate revenue collection.
- Conductcommunity presentations or represent Summit ElderCare at externalfunctions or meetings upon request of the Medical Director orExecutive Director.
- Pursue professional growth through continuingeducation that, at the minimum, meets the Massachusetts Board ofRegistration requirements for license renewal.
- Assist witheducation of PACE staff upon request.
- Perform other duties asneeded.
Qualifications Education:
Medical degree, Geriatricspecialty strongly desired
License:
A doctor of medicine orosteopathy legally authorized to practice medicine or surgery inthe Commonwealth of Massachusetts. Current DEA certificate.
Certification:
Board certification in Internal Medicine,Geriatrics, or Family Practice preferred. Reliable transportationrequired.
Experience:
At least 1 year working with a frail orelderly population.
