LIFE at St. Joseph of the Pines (SJP) has an excellent full-timeopportunity for a Board Certified Geriatric Medicine, InternalMedicine or Family Medicine physician to join as Medical Directorof the Program of All-Inclusive Care for the Elderly (PACE)facility located in Fayetteville, North Carolina. Program of All-Inclusive Care for the Elderly (PACE) model iscentered on the belief that it is better for the well-being ofseniors with chronic care needs and their families to be served inthe community whenever possible. PACE serves individuals who are age 55 or older, certified by theirstate to need nursing home care, able to live safely in thecommunity at the time of enrollment and live in a PACE servicearea. While all PACE participants must be certified to need nursinghome care to enroll in PACE, only about 7 percent of PACEparticipants nationally reside in a nursing home. If a PACEenrollee needs nursing home care, the PACE program pays for it andcontinues to coordinate the enrollee’s care. We encouragefellowship and independence while serving seniors and theirfamilies with dignity and respect. Delivering all needed medicaland supportive services, PACE is able to provide the entirecontinuum of care and services to seniors with chronic needs whilemaintaining their independence in their home for as long aspossible.

Job Description:

Participates in provider on-call rotation as required. Under the supervision of the Executive Director, the MedicalDirector provides general medical direction and supervision of themedical aspects of participant care. Responsible for the delivery of participant care, clinicaloutcomes, and oversight of the Quality Assurance PerformanceImprovement (QAPI) program. Responsible for the development and maintenance of medicalpolicies and procedures/standards and protocols. Provide direction and supervision to related Primary CareProviders (PCPs) and Advanced Practice Providers (APPs). Conduct history and physical on new participants andevent-based and periodic reassessments. Develop medical treatmentplan and provide medically necessary services in the range oflocations in which participants may reside. Interview and evaluate applicants to PACE Organization andtheir caregivers and participate in the Intake and Assessmentdecision. Discuss each participant s medical conditions and prognosiswith him/her and their caregiver to assist them in expressing theirwishes and planning their future care. Execute assignments in a culture that is shared andcollaborative across all divisions within TH PACE. Collaborate in the nursing treatment program ofparticipants. Establish and/or maintain as a member of the leadership team aplan to achieve operating goals. Participate as an active member of PACE Organizationinterdisciplinary team. Educates and advises the team regarding theimplications of diagnoses, cognitive and physical function andtreatment options. Participate in team and family conferences and decision-makingprocesses. Educate participants and families regarding health maintenanceand their chronic medical conditions, in cooperation with nursingstaff. Supervise the health care and treatment program as developed bythe interdisciplinary team. Perform physical examinations, order and evaluate appropriatelaboratory and diagnostic tests, and record results in a systematicmanner. Living Independently for Elders (LIFE) is a Program ofAll-inclusive Care for the Elderly (PACE) at SJP, which assistsolder adults in living safely at home. Eligible participants are55-years old or older, live in the designated service area, meetthe state requirements for nursing home care and are able to livesafely in the community. LIFE is designed around the needs of the individual and providescomprehensive medical, health and social services (including adultday health programs, rehabilitation therapy, nutritious meals andsocial engagement) to participants at the center as well as intheir homes.
