Responsibilities:

Provide oversight and mentorship to our clinicians in the greater Greenville, North Carolina, areaConduct clinical case reviews, share tools, information, and guidelines as they relate to quality of careRepresent our facility in engaging, establishing, and maintaining relationships within the communityProvide clear communication with clinicians, facility leaders and staff regarding care and services for enrolleesCollaborate with leadership to review care coordination initiativesDevelop intervention plans, and share ideas about network management issuesWork alongside pharmacists on medication managementComplete Medicare required visitsComplete acute, follow up, and discharge visits as necessaryImplement an individualized plan of care for patients’ goalsMake recommendations on transitional care, DME, home health services, or outpatient services after short stayFacilitate and assist leadership in quality improvement initiatives and risk managementTimely response in chart completions and pharmacy recommendations•Clinicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), geriatric, primary, internal (IM), adult, post-acute (PAC), home health, hospice, palliative and hospital medicine (HM).Not far from North Carolina’s coast, Greenville is a small city along the Tar River, widely recognized as the thriving cultural, educational, economic and medical hub of Eastern North Carolina.
