Responsibilities:
Provide oversight and mentorship to our clinicians in thegreater Greenville, North Carolina, area Conduct clinical case reviews, share tools, information, andguidelines as they relate to quality of care Represent our facility in engaging, establishing, andmaintaining relationships within the community Provide clear communication with clinicians, facility leadersand staff regarding care and services for enrollees Collaborate with leadership to review care coordinationinitiatives Develop intervention plans, and share ideas about networkmanagement issue Work alongside pharmacists on medication management Complete Medicare required visits Complete acute, follow up, and discharge visits asnecessary Implement an individualized plan of care for patients’goals Make recommendations on transitional care, DME, home healthservices, or outpatient services after short stay Facilitate and assist leadership in quality improvementinitiatives and risk management Timely response in chart completions and pharmacyrecommendations Clinicians trained or practicing medicine in thefollowing disciplines are encouraged to apply: family (FM),geriatric, primary, internal (IM), adult, post-acute (PAC), homehealth, hospice, palliative and hospital medicine (HM).Not far fromNorth Carolina’s coast, Greenville is a small city along the TarRiver, widely recognized as the thriving cultural, educational,economic and medical hub of Eastern North Carolina.
