Job Description

Manchester, NH Elliot Health System (EHS) seeks a hungry, humble and smart (Patrick Lencioni) leader to serve as its next Chief Medical and Transitional Care Services Officer (CMO). This is a newly structured physician leadership role with a growing, nimble, clinically sophisticated independent regional health system approaching $1 billion in net revenue. EHS is led by a physician CEO and has realized strong financial performance in a growing market. The CMO provides executive physician leadership to Elliot Hospital and the Elliot Medical Group focusing on operational, clinical, and performance management. Reporting to the Chief Physician Executive and collaborating closely in a dyad model with the Nursing Vice President of Patient Care Services, the CMO ensures clinical and operational excellence, and strategic alignment across all care settings to fulfill the organizational vision to be “your first choice to give and receive care”. This role also focuses on enhancing care coordination and continuity across the continuum, including pre- and post-acute strategies, transitional care management, and care coordination to support value-based initiatives and distinguished service strategies. In addition to advancing service line development and operational excellence within the acute care and ambulatory setting, the next CMO is instrumental in working with home health, hospice, and other transitional care leaders to optimize outcomes and innovative models of care. Additionally, the CMO leads efforts to develop a cohesive system of behavioral healthcare that meets the diverse needs of the community. The ideal candidate for this role will have a minimum of 8-10 years of clinical and administrative experience, with at least 3-5 years in a leadership role overseeing care services. Experience in hospital-based services, primary care, care coordination, and transitional care management are strongly preferred. A medical degree (MD, DO) from an accredited institution, with a residency in internal medicine or a related specialty, is required, while an advanced degree in healthcare leadership (MMM, MBA, MPH) is preferred.
