Description:
Locally owned Community Hospice is looking to fill the position of
TEAM SUPPORT
in our Foley office.
Job duties are as follows:
Answers and routes telephone calls to the appropriate team member Coordinates communication among team members and provides information or relays messages to patients and team members Collaborates within the interdisciplinary team setting as well as works independently with minimal supervision Completes workflow in the electronic medical record as assigned Prepares, sends and distributes faxes Scans documentation into the patient’s electronic medical record Collaborates with the Administrator to verify all orders and plans of care are properly authorized by the Medical Director in a timely manner in accordance with federal and state regulations Checks, maintains stock and orders medical supplies Orders and faxes confirmation of all Durable Medical Equipment (DME) for pickup on death/discharge of patients; maintain accurate reporting of DME locations at all times Works as a member of special or ongoing projects and other related duties as assigned Faxes orders to Medical Director for signature, to facilities as needed •Performs other duties as assigned
Requirements:
High School Diploma or equivalent Three or more years professional work experience; hospice experience preferred Excellent interpersonal, verbal and written communication skills Ability to work cooperatively with all members of the organization Demonstrated ability to work as a member of a team, with strong organizational skills Extensive computer and database experience, with proficiency in Microsoft Word, Excel, PowerPoint, the Internet and mail merges; prior use of Electronic Medical Records (EMR) preferred Must be able to prioritize duties daily in response to rapidly changing patient care needs; ability to utilize outstanding customer service skills Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order; ability to travel to all business locations for training and/or meetings as required
