Job Description:

• Serves as the key external-facingmember of cross-functional/divisional team of designated resourcesassigned to manage chart abstraction for assigned providers andfacilities
• Informs and educates providers and physicians onClient’s chart collection activities and is responsible forfostering provider relationships and adoption of Client RiskAdjustment initiatives
• Responds to provider inquiries regardingprovider related information and data to resolve issues andincrease chart retrieval volume.
• Educates, monitors, analyzes,documents, and interacts with physicians and other health careprofessionals to ensure cooperation and understanding of chartretrieval projects. Alerts leadership to potential issues andopportunities found through physician contact.
• Maintains athorough knowledge of healthcare industry trends and developmentsthrough the review of industry publications, newsletters and formaltraining.
• Ensures medical records are assembled in standardorders and are accurate and complete Ensure and meet productivityexpectations standards for collection of medical record.
• Maintainstrict confidentiality and security with all patient records whileensuring compliance with HIPPA standards.
• Receive and processchart requests in accordance with the Company Standards.

Ensuresintegrity of chart retrieval database and quality audit controls ofall medical records documents in a timely and accurate manner.
