GENERAL SUMMARY OF DUTIES

: Works in concert with Central Medical Record team members in processing medical record requests for each office.

ESSENTIAL FUNCTIONS

: 1. Organize and manage patient’s health information data. 2. Ensure release of information paperwork is properly completed. 3. Verify accuracy and accessibility of files. 4. Ensure all files are secure. 5. Obtaining proper authorization, release medical information to: Other medical facilities Lawyers Insurance Companies 3rd party Medical Record Retrieval companies Patients 6. Follows medical records policies and procedures including HIPPA and Corporate Compliance. 7. Maintains patient confidentiality. 8. Attend team meetings. 9. Provide monthly medical record completion report to supervisor 10. Meet a specified quota of medical record releases within a workday. 11. Ensure accuracy in processing patient information. 12. Work with office personnel to provide a customer focused effort in providing medical record release services. 13. Assist with development of medical record workflow that meets the need of patients and PHC offices. 14. Answer patient request both by telephone and in writing in a professional, efficient, and courteous fashion. 15. Responds to requests for medical records; processes letters and reports; answers and directs telephone calls. 16. May photocopy records and documents for billing and/or legal services; sends and receives information via facsimile machine. 17. Keeps supervisor informed of problems or issues; monitors supplies needed; performs other duties as assigned. 18. Other duties as assigned.

Requirements:
EDUCATION

High School Diploma or G.E.D. Completion of course in medical records preferred.

EXPERIENCE

Previous experience in medical records preferred. Typing ability of 40 wpm, word processing, and computer experience helpful.

Knowledge of clinical workflow in an ambulatory setting. Knowledge of end-user computer operation such as word processing, spreadsheets & e-mail. Knowledge of customer services concepts. Skill in organizing & prioritizing tasks. Skill in using customer service skills to assist frustrated users. Skill in masterful problem solving. Ability to communicate effectively in user-friendly terms verbally and in writing. Ability to be a team player with effective interpersonal skills. Ability to solve problems and work independently. Ability to work under pressure and deadlines. Ability to act as a public relations agent for clinic. Ability to handle multiple tasks simultaneously and prioritize work needs. Knowledge of modern office equipment Interpersonal/human relations skills Telephone etiquette skills Ability to maintain records and files Ability to maintain confidentiality Ability to exert physical effort maintaining and distributing files

ENVIRONMENTAL/WORKING CONDITIONS

: Office setting, computer facility, and clinical offices. Minimal exposure to communicable diseases.

PHYSICAL/MENTAL DEMANDS

Standing, sitting for long periods while doing computer input and responding to phone questions. Must have ability to bend, stoop, twist and work with computer equipment.
