Incumbent is responsible for performing quantitive and qualitive reviews of health record documention. Responsible for ensuring th all pient care da entered in VistA and/or Computerized Pient Record System (CPRS) is accure, timely, and completed. Adheres to established documention requirements as outlined by The Joint Commission regulions, Veterans Health Administrion (VHA) guidelines, as well as medical-legal requirements. Reviews records for adherence with CMS guidelines reled to student, resident supervision and tending physician presence and documention. Compiles reports to insure provider specific and service compliance. Maintains a control system to ensure completion of all inpient and outpient records in accordance with VA, The Joint Commission, CMS and other regulory agencies standards. Reviews health records to ensure th all records contain sufficient informion to meet medical/legal requirements, to ensure continuity of pient care, and to support educion and research needs. Assists clinicians with completion of delinquent/incomplete health records. Performs the quantitive and qualitive analysis on both computer and paper-based health records. Consults with the approprie clinician(s) for clarificion when conflicting or ambiguous informion appears in the health record documention. Identifies health record deficiencies, tracks deficiencies and ensures th the approprie individual completes their deficiencies. Assists physicians with the completion of delinquent/incomplete health records and serves as the expert resource for clinical staff regarding requirements for complete documention. Responsible for expediting the completion of the health record when the pient is transferred for care another medical center, for legal requests, regulory agencies review, and reimbursement purposes. Assesses the paper health record and electronic Computerized Pient Record System (CPRS) for completeness and notes deficiencies. Tracks electronic signures and documents deficiencies and communices these deficiencies to clinical staff for correction. Notificion can be in person, internal e-mail, telephone, or through other normal routing system. Works independently resolving health record deficiency issues/erroneous documents and corrective action required within CPRS and only seeks guidance of immedie supervisor and/or Clinical Applicion Coordinor when issues surface when no precedent or procedure has been developed. Prepares complex weekly and monthly reports noting the stus of all incomplete/delinquent records and compliance reports and submits the reports to the approprie staff. Preforms analytical tasks associed with the resolution of “filing” errors on reports uploaded by a transcription vendor. This includes correctly identifying the pient the report belongs too and filing/assigning it correctly. Duties includes the generion of various stistical reports to track these errors. Makes approprie recommendions for changes or solutions and implements approprie recommendions for corrective action when authorized so continued filing errors do not occur. Identifies reviews, verifies, and corrects all stistical da concerning medical transcription to maintain accuracy of medical and stistical reports. Compares and resolves discrepancies between daily Gains and Losses Sheet, Digital System Workload Report and contract transcription logs as it reles to timeliness and accuracy of dicted reports completed by medical transcription contractor. Reviews health records for adherence with guidelines reled to resident supervision and tending physician presence. Works with interdisciplinary staff to solve problems regarding proper health record documention and completion. Develops and compiles written reports trending provider specific, service, and hospital compliance reports regarding admission subsequent and discharge notes. Obtains stistical informion, applies stistical formulas, and completes stistical reports for medical center management. Prepares weekly and monthly reports noting the stus of all incomplete/delinquent health records and submits the reports to the approprie administrive and clinical staff. Participes in routine and focused reviews for the purpose of identifying problems and potential problems in record documention as part of the health record review process. Identifies and abstracts informion from health records for special studies and audits both internal and external.

Six months of the required one year of creditable experience must have provided the knowledge, skills, and abilities (KSAs) needed to perform MRT HIT work. Educion . An associe’s degree from an accredited college or university recognized by the U.S. Department of Educion, with a major field of study in health informion technology/health informion management, or a reled degree with a minimum of 12 semester hours in health informion technology/health informion management (e.g., courses in medical terminology, anomy, physiology, legal aspects of health care, and introduction to health records). Experience/Educion Combinion . Equivalent combinions of creditable experience and educion are qualifying towards meeting basic experience requirements. The following educional/training substitutions are approprie for combining educion and creditable experience: (a) Six months of creditable experience th indices knowledge of medical terminology, privacy and release of informion, the health record, and one year above high school with a minimum of six semester hours of health informion technology/health informion management. (b) Six months of creditable experience th indices knowledge of medical terminology, privacy and release of informion, the health record, and successful completion of a course for medical technicians, hospital corpsmen, medical service specialists, or hospital training obtained in a training program given by the U.S. Armed Forces or the U.S. Maritime Service, under close medical and professional supervision, may be substituted on a month-for-month basis for up to six months of experience, provided the training program included courses in anomy, physiology, and health record techniques and procedures.

May qualify based on being covered by the Grandfhering Provision as described in the VA Qualificion Standard for this occupion (only applicable to current VHA employees who are in this occupion and meet the criteria) All persons employed in VHA as a MRT HIT, on the effective de of this qualificion standard, are considered to have met all qualificion requirements for the grade held, including positive educion and licensure/certificion/ registrion th are part of the basic requirements of the occupion.
