Position Summary:

The Medical Records Clerk performs a variety of clerical duties involved in maintaining patient medical records to include filing, retrieving, and distributing medical records and reports in accordance with established policies and procedures, auditing patient files and flagging appropriately for errors and missed signatures, etc.

Qualifications:

Education :

Must have high school diploma, preferably a bachelor’s degree. Certification in Medical Records preferred. Will be required to maintain certifications in CPR and CPI. Work experience : Medical Records experience required. Certification in Medical Records preferred.

Knowledge, Skills & Abilities :

Must have excellent telephone etiquette; strong communication skills (oral and written); positive people skills. Must be computer literate. Completes assignments accurately, in a timely manner, and within procedural guidelines. Organizes and prioritizes work.

Interpersonal Skills :

Must be a team player with tact and cooperativeness; take initiative; have excellent organizational and time management skills; able to handle fast-paced workload; ability to work in an environment with frequent interruptions. None. Essential Job Duties % of Time 1. Assures the integrity, completion, and authenticity of documentation. Sort, file and collate in appropriate sequence and in accordance with established procedures a variety of medical record information needing to be scanned into patient records. Complete all required chart audits – examine patient medical records for completeness, ensuring all required information is included; note any deficiencies and refer to appropriate person for follow-up. 60% 2. Usage and release of information in medical records per policies and regulatory standards. Responds to request for medical records and release same to authorized requesters; retrieving and copying patient medical records information in support of improvement activities performance improvement activities or regulatory audits; records to answer authorized requests; document releases in accordance with established policies and procedures. Maintain daily census lists and discharge logs as requested; check census information against existing records for accuracy and completeness, verifying information and making corrections as necessary. Prepare routine statistical reports from existing records reflecting program activities. 20% 3. Filing, storage, and retrieval of medical records Maintain accurate master medical records file storage index. Request medical records files as needed tracking records location history. Establish and maintain check in and checkout system for records. Keep records in secured, double locked areas at all times. 15% 4. Any other duties as assigned. 5%
