Job Summary:
The Referral/Procedure Specialist assists in coordinating referrals to physicians, post-acute care service (home health, hospice, etc.), hospitals, and imaging centers. In addition, the Referral Specialist is part of a Patient Centered Medical Home (PCMH) and is expected to be familiar with the philosophy, goals and objectives of the PCMH.
Duties & Responsibilities:
Initiates all routine non urgent referrals/procedures within 5 business days. Initiates all urgent or STAT within the next business day. Ensures that referrals/procedures are accurate and completed and scheduled in a timely manner. Ensuring that provider’s referral/procedures orders are prioritized, and authorization competed as needed, and that the patients are made aware of appointment. Track referrals/procedures according to the urgency of the referral: within 10 business days from the expected completed date if a consultation report, notes, or other documentation not received from the referred to clinician or facility for all urgent referral/procedures orders. Track referrals/procedures according to the urgency of the referral/procedure: within 15 business days from the expected completed date if a consultation report, notes, or other documentation not received from the referred to clinician or facility for all routine referral/procedure orders. Utilizes established documentation system to notify the referring provider if patients do not adhere to the recommended plan of care and/or authorization is not approved. Demonstrate the ability to establish and maintain effective, productive relationships with co-workers, supervisors, patients, and the public. Performs other duties as assigned.
Required Education, Certifications, Skills & Abilities:
A High School diploma required.
Medical Office experience:
2 years preferred. Knowledge of medical terminology required. Must be flexible to the changing needs of the center. CPR certification recommended.
Additional Requirements:
Must possess valid driver’s license. Must be willing to work with SMF EHR. Must be willing to work at different SMF locations if requested.
Training Requirements:
The Referral/Procedure Specialist will be responsible for attending all mandatory training including evidenced based approaches to self-management support, proactively addressing the needs of patients and families in this community, cultural competency, effective communication, care coordination, OSHA, HIPAA, and EHR, etc.
Physical Requirements:
