QUALIFICATIONS

Two years of experience in a health care environment such as a hospice agency, a home health agency, or physician’s office in either a clerical or clinical role. Must have an understanding of the importance of precise documentation in a patient’s medical record. Demonstrated ability to prioritize tasks and handle pressure and delicate situations. Demonstrated understanding of customer relations, tact and diplomacy in facilitation of relationships with staff and the public. Demonstrated knowledge of computer usage and word processing. Recent experience with medical records system in home care/hospice agency preferred. Demonstrated knowledge of medical terminology. Demonstrated ability to perform detailed work in a timely manner with accuracy and thoroughness. Demonstrated ability to respond well to supervision and to work independently when appropriate. Demonstrated excellence with both written and verbal communication skills.

RESPONSIBILITIES

(including but not limited to): Prepare and maintain master patient list and daily census. Structure and maintain files of all necessary chart forms. Documents are filed in medical records within (24) twenty-four hours of receipt. Track submission, receipt and filing of plans of care, histories and physicals, physician orders, certification statements, recertification and other chart documents as appropriate. Provide information to corporate billing department on admissions, discharges and status change. May receive initial information regarding patients by telephone, complete required forms and relay information to nurse/team coordinators. Maintain accurate physician orders logbook. Order monthly medical and office supplies. Send and track returned Patient Satisfaction Surveys. Prepare and send out patient satisfaction survey questionnaires according to policy and procedures. Review records for missing information and return to the PCC for completion. Track patient visits by comparing visits to schedules daily. Review time sheets for accuracy. Abide by all Unity Hospice Care medical record and confidentiality policies and procedures. Perform other duties as requested.
