The Medical Scribe works directly under the supervision of the provider to facilitate patient flow by performing accurate documentation/ patient encounters into the Electronic Health Record ensuring that medical records are completed and up-to-date, gathering information for the patients visit and partnering with the provider to deliver efficient patient care.
Responsibilities:
Documenting into electronic medical record of the medicine, history of present illness, physical exam, diagnostic results, consults and discharge. Will accompany providers into the patient room to transcribe the physical exam findings into the electronical medical record. Will update family history, social history, past medical history, medications, allergies and immunizations into the chart. Will access past medical records for the provider. Entering orders at the direction of the physician. Assist patients in answering structured questionnaires and recording their responses. Collaborate with physician in developing templates for documentation. Record review and data collection and tabulation for purpose of quality improvement Completing patient forms using information available in the medical record Obtains verification of patients’ insurance benefits for initial and subsequent treatment; completes and distributes necessary verification forms. Provides support to Physicians, as needed. Maintains all office equipment or arranges for such care. Acts as a liaison between staff and referral sources. Contacts physicians’ offices concerning requested services. Prepares the chart on each patient by obtaining all necessary forms, information and prescriptions and keeping them up-to-date. Perform other duties as assigned or needed. Maintains confidentiality of patient information. Adheres to policies and procedures specific to patient rights. Proficient in basic computer operations and all office automation equipment (telephones, voice-mail, desktop computing, electronic mail, facsimile procedures, automated scheduling). Adheres to policies regarding attendance, conduct, grooming and dress code. Exhibit sound organizational skills and personal initiative. Exhibit professional demeanor and communication skills. Knowledge of medical terminology and insurance verification. Proficient with medical authorization for services. Exhibit the ability to communicate with a wide variety of individuals (patients, families, physicians, attorneys, external funding agencies, allied health workers, and co-workers). Promotes a positive image of all companies inclusive of Complete Practice Management Solutions (this includes their associated and subsidiary companies) to outside agencies and the public. Participate in performance improvement activities as needed. Exhibits excellent service to patients, visitors, physicians, and co-workers. Shows courtesy, compassion and respect. Documents observations and care given according to hospital and office policies and procedures. Observes, records and reports subjective and objective patient symptoms. Takes vital signs, temperature, pulse, respirations and blood pressure. Attends in services to further personal and job-related development. Maintains confidentiality of patient information Transcribe patient encounters and document medical histories during examinations. Maintain accurate and thorough medical records in compliance with legal and ethical standards. Ensure timely entry of medical documentation into electronic health record (EHR) systems. Stay updated on medical terminology, anatomy, and physiology to enhance documentation accuracy. Support the healthcare team in various administrative tasks as needed. Qualifications:
High School diploma or equivalent Medical assistant training preferred Ability to organize and prioritize work and manage multiple priorities Excellent computer skill Excellent verbal and written communications skills Excellent attention to detail Ability to research and analyze data Ability to work independently with minimal supervision Ability to establish and maintain effective working relationships with providers, management, and staff Must be able to communicate clearly to patients in person and over telephone Requires occasional lifting and carrying items weighing up to 20 pounds unassisted. Requires frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
