- Verifies identity of resident receiving medication.
- Ensures that each resident’s Medication Administration Record matches the order as written by the physician. Follow order as written, double check. Med to Mar to cart each med pass.
- Verifies prescription order reads the same on the MAR and prescription package.
- Assists in administering medication as prescribed by the resident’s physician and documents the medication received in the resident’s Medication Administration Record (MAR) as indicated on 1823 Form. Documentation includes name of drug, dose taken and time administered.
- Wears gloves when assisting with medications.
- Observes ingestion or other application and initials MAR.
- Signs the MAR after passing medications to resident.

- Notes any adverse actions or side effect of the medication.
- Ensures adequate ordering, receiving, documentation, and safe storage of physician-prescribed medications for residents.
- Monitors inventory of medication and orders new medication or refills from pharmacy, VA, etc.
- Ensures that all medications on the residents MAR are in the appropriate resident’s bin.
- Counts narcotics at the change of each shift.
- Communicates verbally as well as documents in communication log at the end of each shift any hospital admissions, behavior problems or medication issues.
COMMUNICATION IS VITAL TO THE SMOOTH OPERATING OF THE NURSE’S STATION.
- Ensures medications are refilled daily as needed by communication with pharmacy staff.
- Faxes prescription order to pharmacy and places a copy of prescription in the Prescription Book. Once medication is received, the copy is to be removed from the Prescription Book.
- Maintains medication cart for cleanliness.
- Completes appropriate progress and observation notes.
- Checks bins for discontinued medications.
- Performs routine weights on residents and documents results.
- Notifies Administrator, Assistant Administrator, Resident Care Manager and/or Resident Care Coordinator of any noted resident health changes.
- Assist healthcare professionals in providing patient care and treatment.
- Maintain accurate and detailed records of patient information and test results.
- Ensure the proper handling and storage of medical equipment and supplies.
- Follow established protocols and procedures for infection control and safety.
- Collaborate with other members of the healthcare team to ensure efficient and effective patient care.
- Use electronic medical record systems such as ECP to document patient information. Additional Duties and Responsibilities
- In Absence of Administrator/Managers
- Responds to resident concerns including injuries, contacting 911 when appropriate.
- Notifies family members, administrators, supervisors and case manager when a resident is transported to hospital, documenting who was informed.
- Maintains contact with hospital when resident is transferred.
- Serves as liaison between residents and physicians, home health care agencies, pharmacies and ancillary services.
- Completes incident reports, including fall and skin tear report.
Assists in ensuring that residents receive care in a manner and in an environment that maintains or enhances their quality of life. Performs all other tasks as requested. 2nd
SHIFT MEDICATION TECHNICIAN
- Files all left over orders.
- Reviews prescription book to ensure medications are received. o If received, files prescription. o If not received, refaxes request form with prescription to pharmacy, documenting time, date and name on the request.
- Writes observation notes
- Ensures that all charts are in alphabetical order.
- Orders any refills via fax to the pharmacy. o Documents date, time and name on the refill request form and places form in the prescription book.
- Audits charts for comparison of medications on cart to MAR, following list.
- Removes any discontinued medications.
- Reviews MAR for missing signatures and places need for corrections in communication log.
- Leaves a report for Administration/Managers in communication log.
- Passes PRN medications.
- Counts all narcotics. Minimum Education/Experience Qualifications
- Proficient in understanding of written and spoken English is required.
- Current Medication Technician Certificate. 6 hours of initial training
- Two hours of update training annually
- 1-2 years’ experience as Medication Technician in assisted living, long-term care facility, personal care facility or similar setting is preferred.
- First Aid and CPR certification.(Preferred) Knowledge/Skills/Abilities
- Solid decision making and problem solving skills.
- Ability to carry out successful communication with resident as well as administration, physicians and pharmacy.
- Ability to develop rapport with geriatric population, families of residents, and team members.
- Strong written and verbal communication skills.
- Ability to project a calm, professional demeanor under stressful situations.
- Thorough knowledge of federal and state regulations governing assisted living communities.
- Proficient in use of computer and Microsoft office software.
