JOB DESCRIPTION

Overview The Supported Living Coach plays a vital role in assisting individuals with developmental disabilities and autism in achieving their personal goals and enhancing their quality of life. This position involves providing direct support to clients in their home and community settings, fostering independence, and promoting positive behavioral health. The ideal candidate will be compassionate, patient, and skilled in effective communication, ensuring that each individual receives personalized care tailored to their unique needs. Responsibilities Provide direct support to individuals with developmental disabilities and autism in daily living activities. Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors. Collect and maintain accurate data on client progress and behavior for reporting purposes. Facilitate community integration by supporting clients during outings and social activities when needed. Communicate effectively with clients, families, and interdisciplinary teams to ensure continuity of care. Support clients in achieving personal goals through individualized care plans. Experience Experience working with individuals with behavioral health challenges or developmental disabilities is preferred. Knowledge of home community care practices is beneficial. Skills in data collection and documentation are essential for tracking client progress. Previous experience in social work or related fields is advantageous. Familiarity with behavior management techniques is a plus. Strong communication skills are necessary for effective interaction with clients and team members. Experience working with individuals on the autism spectrum is highly desirable. Proficiency in American Sign Language is a valuable asset. Pay:
